1. Do you have fever, new onset of cough, worsening chronic cough, shortness of breath, or difficulty breathing?
2. Did you have close contact with anyone with acute respiratory illness or travelled outside of Canada in the past 14 days?
3. Do you have a confirmed case of COVID-19 or have had close contact with a confirmed case of COVID-19?

If you answered "yes" to any of these questions, you should not be seen at our office. Instead, you should call your primary care provider or Telehealth Ontario at 1-866-797-0000.

If you have an urgent or emergency eye health condition, you should be referred to an on-call ophthalmologist by your primary care provider, proceed directly to the emergency room at your nearest hospital or urgent care centre or managed via telehealth if possible.
